Information on this website
You can browse these pages without entering personal information. Our contact form asks for your first and last name, email address, inquiry selection, and message. The same form is used for appointment inquiries and other questions.
Cookies and other tracking
These pages do not set cookies or save form information in browser storage. Form entries remain temporarily in the open page until you leave or reload it. Your selected color theme is saved in browser storage so it can be remembered between visits. Form entries are not stored there. Your browser may offer its own autofill features. They do not include advertising pixels, third-party analytics, session recording, embedded maps, social feeds, or chat widgets. Images, styles, and scripts are served with the website.
Hosting and communication
When a website is requested, hosting and network providers process technical information such as an IP address, browser information, and the requested page to deliver and protect the service. Server logging and retention depend on the hosting provider and its configuration.
When you select Send now, the completed form is sent to this website’s server and emailed to info@pacmeridianmed.com. The form does not save your message in a website database. Hosting and email providers process the message for delivery, and the practice receives it in its email account. The server temporarily keeps a hashed network address and submission counts to limit repeated submissions; these records expire after ten minutes and are removed during a later submission. Clicking a phone or email link opens the relevant application on your device.
